Things to bring to your first visit
- Your referral slip and x-rays from your referring dentist. Many times your dentist will send us both of these electronically and you won't need to bring them. But if you have them, bring them!
- Prior to your first visit, you will be emailed and or texted a link to complete paperwork. It is important this paperwork is completed prior to your appointment and NOT during your first appointment. Having your paperwork completed ahead of time will allow Dr. Antoine the most time with you during your visit.
- Please bring an up-to-date list of medications to your appointment. We will review your medication list with you and scan it into our system rather than having you list each medication on your medical history.
- If you have dental insurance, please bring any forms or insurance cards with you to the appointment.
Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension).
We will complete an in-depth medical and dental health history and a thorough examination in your mouth and work closely with you and your dentist to develop a plan to meet your wants and needs.
